Health Information

Thomas MacLaren School is committed to your child's health and wellness. We are available to respond in emergencies and for all the minor bumps and bruises incurred on the campus. We also evaluate students who are feeling ill to see if they need to go home, or if we can support them with treatment and/or rest, to make it through the day. We also help coordinate contact with teachers if a student is going to be out for an extended time.

MacLaren Illness Guidelines: Please use this information to decide whether or not your child should stay home from school.

To help us better care for each student, we ask parents/guardians to submit an annual health update, including latest vaccination records or exemptions, and health plans (see below), before the start of each school year. 

The Annual Health Update form is part of the electronic Student Update packet that is emailed to every family to fill out as a part of the yearly Registration process. 

Vaccination records/exemptions can be uploaded as part of the online process.  We will accept records from your health provider's office or from your previous school.

Vaccination records/exemptions that are not uploaded at registration time can be submitted in the following ways:

  • Faxed using our Secure Fax number - (866) 587-2608
  • Hard copy mailed or hand-delivered to the school

Authorization to Administer Medication: For any student requiring medication during the school day, the Authorization to Administer Medication form must be completed before any medications can be administered. This form must accompany EACH medication, INCLUDING over-the-counter medication. It must be completed by the parent/guardian and signed by a health care provider with prescriptive authority.

Students with Severe Food Allergies or Intolerances: If your child has severe food allergies or intolerances, please read these important guidelines that we use at MacLaren.

Heath Plan Information: If your child has the following health issues, the requisite forms below must be completed and submitted before the first day of school. Forms may be submitted via fax via using our secure fax line (866) 587-2608, or by delivering them to the school in person, or by mail. Please mark them to the attention of Karen Triplett.

  • Asthma: If your child has asthma and has a prescription to carry medication (even if it is not all the time), you will need to complete the following forms, have them signed by you and a health care provider with prescriptive authority, and submit before the first day of school:
